POLICY STATEMENT

Paramount Health Options (“PHO” or “we”) is committed to respecting and protecting the privacy of its website guests. This privacy policy (“Privacy Policy”) describes how PHO handles the information that you provide to us on this web site (the “Web Site”). This Privacy Policy applies only to the Web Site, not to any other PHO or third party web sites that may be accessible or linked to from here.

Please be sure to carefully read this Privacy Policy before using the Web Site or submitting information to us. By (a) using any part of the Web Site; or (b) submitting information via the Web Site, you consent to the collection, use and distribution of such information by us for the purposes described in this Privacy Policy.

1. Information We Collect From You
 

Active Information Collection. PHO does not collect personal information about you (like name, email, address, contact information, preferences) unless you voluntarily submit it to us. For example, we may ask you to provide personal information to us to register with the Web Site. Some of this information may be uniquely identified with you, such as your name, business name, address, phone number, email address, etc. We only use this personally identifiable information for the purpose you provide it to us (e.g., sending you information, processing your payment, etc.).

Passive Information Collection. As you utilize the Web Site, certain information can be passively collected (that is, gathered without you actively providing the information) using various technologies and means, such as navigational data collection. Like most web site operators, we track IP addresses during your visit and use session, persistent, and flash cookies and may use any other successor tracker technology. Our system also automatically gathers information about your use of the Web Site and collects operational information about the technology you use, such as your browser, type of computer, operating systems, and Internet service providers.

We passively collect and use information to generate statistics and measure Web Site activity in order to understand what areas of the Web Site are most popular, which areas may require improvement and what technologies our visitors are using. This information helps us to update and improve the Web Site and enhance the usefulness of customer visits. 2. How We Use Your Information

PHO may disclose your information to our employees in order to process your requests or our business partners in correlation with group purchasing opportunities for the purpose of providing services to you. Our business partners will be given limited access to your information as is reasonably necessary to deliver services to you. We use contractual or other measures to restrict such partners and service providers from using your personally identifiable information in any manner other than as may be necessary to perform their services and to provide a level of security and privacy protection comparable to the security and privacy protection measures we enact regarding your personally identifiable information.

Payments to PHO are made through a secure, independent third party payment processor. PHO will not have access to any credit card numbers with payments made through our website. PHO will be given payment amounts, names, addresses and corresponding information (invoice number, comments, etc.) of the credit card holder for processing internally.

We may send you information about features that we believe you may be interested. We also may use your contact information for marketing and other purposes. If you want to opt-out of these emails, you may click the “Unsubscribe” link at the bottom of every email or email us directly at Info@ParamountHealthOptions.com.

In addition, we may provide aggregate statistics about our customers, sales, online traffic patterns and related information to third parties, but these statistics will not include any personally identifying information.

In the event of a sale, merger, liquidation or reorganization of PHO or in connection with a transfer of the assets related to the Web Site, we may transfer some or all of the information we maintain on you to the applicable successor(s), but such transfer will be subject to the terms of this Privacy Policy.

We also will disclose any information that it is required to be disclosed by law.


3. Choices You Have About How We Use Your Personally Identifiable Information
 

We strive to provide you with choices regarding the personally identifiable information you provide to us.

You may contact us at Info@ParamountHealthOptions.com to request access to, correct or delete any personally identifiable information that you have provided to us. To protect your privacy, we will only respond to email messages sent from the email account you specified when you gave us your contact information. Should you elect to have your information deleted, we will also delete your account.

You may opt out of having your email address used for promotional purposes by the Company. This opt out does not apply to information provided by the Company as a result of a purchase or other transactions. If you no longer wish to receive information or promotional materials from us by email you may opt out of receiving these communications by using one of the following methods:

    1. Send an email to Info@ParamountHealthOptions.com asking to be omitted from future email distributions.
   2. Follow the instructions contained in the promotional email or newsletter.

If your browser allows you the following options, you may set your browser to refuse cookies or to alert you when cookies are being sent. If you refuse cookies, please note that some parts of the Web Site may then be inaccessible or not function properly.

4. How We Protect Your Information
 

We work hard to protect your personal information. We have both technical and procedural security measures in place to protect personal information under our control against the loss, misuse and alteration, including but not limited to, SSL and encryption for data transfer, firewalls, restricted access to data, employee training, and other methods. Only those employees who need access to your information in order to perform their duties are allowed access. However, no internet transaction is completely secure and we do not guarantee that data transmissions cannot be accessed, altered, or deleted due to firewall or other security software failures.

The safety and security of your information also depend on you. Never share your password with anyone else and notify us promptly if you believe your password security has been breached.
 
5. Third Party Websites
 

The Web Site may contain links to other web sites operated by third parties that may be of interest to you. We cannot control these third party web sites, which may collect personally identifiable information from you. We have no control over, and are not responsible for, the privacy practices or content of such web sites. When you follow a link and leave the Web Site, you do so at your own risk.

We do not have control over the privacy policies applied by any other party to whom you may disclose information. We are not responsible for their content or privacy policies. If you would like information on another party’s privacy policy, you should contact that party directly, and we encourage you to do so.


6. What We Do About Children Under the Age of 13 Using the Web Site
 

The Web Site is not intended for children under the age of 13. We will not knowingly collect information from visitors in this age group. We encourage parents to talk to their children about their use of the Internet and the information they disclose online. If a child has provided us with personally identifiable information, a parent or guardian of that child may contact us via email at Info@ParamountHealthOptions.com if they would like this information deleted from our records. We will use reasonable efforts to delete the child’s information from our databases.
